Purpose of the Form

This form allows you to:

1. Access your personal data held by the hospital.

2. Correct or update inaccurate/incomplete data.

3. Delete or anonymize data (under PDPA conditions).

4. Withdraw consent for data processing.

5. Request data portability (transfer data to another organization).

How to Submit

1. Complete the form and submit it to the Hospital’s Data Protection Officer (DPO).

2. Provide valid identification (e.g., ID card, passport).

3. The hospital will respond within 30 days of receipt.
