Purpose of the Form

This form allows you to:

  1. Access your personal data held by the hospital.

  2. Correct or update inaccurate/incomplete data.

  3. Delete or anonymize data (under PDPA conditions).

  4. Withdraw consent for data processing.

  5. Request data portability (transfer data to another organization).

How to Submit
